Camper Registration

Camp is right around the corner, August 7th-11th, at Cornerstone Conference Center. If your child is six years of age or older, make plans now for him/her to attend. Children younger than six years of age MUST be accompanied by and adult/parent/guardian at all times. In this packet, you will be asked to designate who may pick up your child. Campers may be dropped off from 1:00-2:00pm on August 7th at the camp. On the last day of camp, August 11th, all campers will be checked out through their Counselor between 9:30 a.m. - 10:00 a.m. This procedure helps us to ensure that all children are released to the correct persons. We are also asking campers to leave all games, toys, and audio/visual devices at home (iPads, iPods, cell phones, etc.). There are two reasons for this; first, we do not want any valued possessions lost at camp, and second, we do not want our campers distracted from the purpose of camp. Absolutely NO CELL PHONES will be allowed for campers/youth. All campers’ personal belongings will be inspected at check in. Anything deemed unnecessary will be withheld and returned to them at the end of the week. Pranks of any kind of prohibited. Failure to follow the no prank guidelines may result in dismissal from camp. The price of camp ($150.00) is non-refundable. Cornerstone Conference Center, where camp will take place, is set in a natural environment. If your child is allergic to the sun or is severely allergic to dust or grass, he/she may not be able to fully participate in camp activities. You may want to consult your child’s physician for advice. Also, any child that is taking medications must be able to self-administer their own medications. If unable to do so, there should be a responsible adult to accompany the child, and the adult must have written permission to assist that child. (Written note should be placed with the child’s folder upon arrival at camp.) As you pack for camp, remember that while the days are rather warm, the evenings tend to be cool. We would recommend packing a sweatshirt or warm pajamas. We also have a dress code that all campers must abide by. Woman/girls/teens are to wear shirts and skirts/dresses below the knee and MUST be modest in nature (i.e. length and size), at all times while outside their cabin/dormitory. No sleeveless dresses/shirts or tank tops. Men/boys/teens MUST wear pants and a shirt at all times while outside their cabin/dormitory. No clothing with obscene gestures or slogans will be allowed. Campers will be allowed to wear knee length shorts/swim trunks while participating in the lake activities. We are anticipating a great year! Parent/Legal Guardian Consent

I, the parent or legal guardian of the camper listed above, do hereby state that I have legal custody of the child who resides with me and have read and approved the included camp information. I give my permission for my child to attend camp and participate in all activities. In consideration of my child being allowed to participate in the event, I authorize West Asheboro Church of God to use my child's likeness in photographs or video in any and all of its publications and in any and all other media. I will make no monetary or other claim against the church for the use of such photographs or video. I give my permission for authorized camp personnel to inspect my camper's belongings to insure that they have no illegal or prohibited items. I will be held responsible for any and all damages my child may cause to the camp site or facilities. I hereby authorize any authorized camp personal to obtain any medical care if necessary. I authorize emergency treatment in the event of an illness/injury when parents are not immediately available. I understand, if necessary, the camper will be taken to a nearby hospital and will be attended by a physician on call. I further understand that I will be responsible for any medical expenses incurred.



Medical History



West Asheboro Permission Waiver

It is my understanding that participating in transportation, programs, recreational and other activities provided by West Asheboro Church of God is a privilege. Prior to my participation or my child(ren)'s participation in such transportation and activities, I recognize there are certain risks associated with the transportation and activities, including but no limited to physical injury due to activity-related accidents, physical injury due to the transportation-related accidents, illness, or even death. In addition, i recognize and acknowledge that there may be other risks inherent in these activities of which I may not be presently aware and hereby do voluntarily assume any such risks. By signing this Permission/Waiver form, I expressly warrant that the child name above or I (if I am a participant) am capable of withstanding both the physical and mental demands of the transportation and activities. I also expressly assume all risks concerning my child or my participation in the activities, whether such risks are known or unknown to me at this time. I further release West Asheboro Church of God from any claim that my child or my participation in the transportation or activities. This release of liability shall include (without limitation) any claims of negligence or breach of warranty. This release of liability is also intended to cover all claims that members of the child's or my family or estate, heirs, representatives, or assigned may have against West Asheboro Church of God. I also further agree to indemnify and hold harmless West Asheboro Church of God from any and all claims arising from my child's or my participation in any of the aforementioned activities and programs, or as a result of injury or illness of my child or myself during such transportation and activities.

Financial Information
